This form accepts Toy requests from families, individuals, and Nor-Profit Organization. Unless otherwise noted, the campaign accepts toy requests for children in the age groups provided.

I agree to the following terms:
- I agree I am the parent or legal guardian of the children for whom I am requesting toys.
- Toys donated by Toys for Tots will not be auctioned, sold or otherwise turned over for monetary donations.
- Toys donated by Toys for Tots will not be taken out of the county for distribution.
- I will not request toys from multiple distribution sites.
- I will not request toys under multiple names/contacts for the same children.
I understand any violation of the above agreement will result in disqualification of current and future donations.
We are seeking your help to make this program as successful as possible and fair to all children. Please do not inflate the number of children on your toy request form. We require proof of parental or legal guardianship for each child. Requests made to multiple centers costs the program valuable time that could otherwise be spent on collecting and distributing toys and will result in disqualification of current and future donations.
